Patient Information:
Appointment Scheduling: At the time you book your appointment we ask that you check your calendar. Please select a day and time when you will have sufficient time. The initial intake appointments for any service takes 1hr 15 minutes. Follow up appointments take 1 hour. Please arrive 15 minutes before your scheduled appointment.

Payments for the appointments are made when the appointment is scheduled.

Please call the office 48 hours in advance if you must change the date and time of your appointment. Only one change is permitted. If your appointment is within 48 hours, no change (or refund) is possible. If your appointment is on a monday, any change must be made no later than the friday prior to your appointment. You cannot change a monday appointment on the preceding saturday or sunday.
