-Packages of appointments are booked and paid in advance. THERE ARE NO REFUNDS FOR PACKAGES OF APPOINTMENTS. BE SURE OF YOUR NEEDS AND SCHEDULES BEFORE MAKING THIS COMMITMENT.

-Please choose your date and time carefully and have sufficient time. If you must make a change, CALL 48 HOURS IN ADVANCE from 9am to 5pm. If your appointment is on a Monday or Tuesday, any change must be made no later than the Friday prior to your appointment. You cannot change a Monday or Tuesday appointment on the preceding Saturday or Sunday. If a sudden emergency keeps you from keeping your appointment, you may reschedule at a $35 penalty.

-You can reschedule an appointment only ONE TIME.

-Each treatment will be 45 minutes. You must arrive at the office 15 minutes prior to the appointment to complete and sign a report of your progress on your treatment plan.

-If you arrive late for your appointment, you will be charged for the additional time for the treatment. (Every 20 minutes $35)

-If you have not followed your treatment plan, the therapist reserves the right to terminate your session and/or stop service. You may reschedule at a later date, but at a $35 penalty.
